What Information Do I Need To CompleteThe Subscription Form?

The subscription form must be completed with all the essential data and information requested. Basic personal and medical information (e.g. biodata, contact, next of kin, pre-existing ailment/past medical history) are required for records, production of I.D cards and also contact (s ) in case of emergencies. 

The form is expected to be filled out, passport photographs of members (with name of members written on the backside) should be attached and submitted to RODING HEALTHCARE via your HR, email, postage, our website or RODING HEALTHCARE office nearest to you. 

Please note that you only need to complete the first provider column if you and your family are to use the same provider. Please fill in all the requested information carefully to avoid a delay in the production of your ID card.

What Do I Do If I Have Registered But My DependentsAre Yet To Register E.G Spouse, Child Etc?

Simply obtain another subscription form, tick the additional membership column, fill in the other necessary details, attach the passport photograph of the dependant(s ) (with name of dependant(s) written on the backside) and send the form through your HR, by post, email or in person to RODING HEALTHCARE.

What Do I Do If IRegistered A LongTime Ago And I Have Not Received My ID Card?

What Do I Do If I Was NotAttended ToIn The Hospital Despite Presenting An ID Card?

If I Want To ChangeMy Provider, What Do I Do?

Simply send us an email or a letter with your details or download the change of provider form from our website and fill it ; the change would be effected immediately as long as the provider is on our network. For providers not on our network you would need to give us at least a month to accredit the provider and get back to you.

If I Am Not Satisfied With TheServices Of A Provider, How Do I Lodge My Complaints?
